Participation Questionnaire

Embarking on a new fitness regime is a great way to stay healthy and age well. 

For some people it is important to first check with your doctor if it is safe to become more physically active and this short questionnaire highlights when it is necessary to consult your doctor.

Please read the seven questions below carefully and answer honestly either YES or NO to each one.

If you answer YES to any of the questions, please consult your doctor and confirm with them what activities are safe for you to do.

If you answer NO honestly to all of the questions, you should be able to safely start becoming more active.

 

Please note Pearla Pilates requires this questionnaire to be completed and any necessary medical consent is received before you start Pilates.

 

1)  Has your doctor ever said that you have a heart condition and that you should only do physical activity recommended by a doctor?

 

2)  Do you feel pain in your chest when you do physical activity?

 

3)  In the past month, have you had chest pain when you were not doing physical activity?

 

4)  Do you lose your balance because of dizziness or do you ever lose consciousness?

 

5)  Do you have a bone or joint problem (for example back, knee or hip) that could be made worse by a change in physical activity?

 

6)  Is your doctor currently prescribing drugs for your blood pressure or a heart condition?

 

7)  Do you know of any other reason why you should not do physical activity?
